"Although though rare, such accidents can occur, and one should be careful to select a skilled and experienced instructor , who places safety above all else."
Sage advice.
2 tandem fatal accidents 1000s of miles apart within weeks of each other, and both sharing some similarities. Both resulting in the death of a young woman, both failed to properly hook-in the student, similar years of flying experience for both pilots (12 and 16 years).
Please let's not see a third. _________________ Mountains always have the right of way.
If you've never forgotten anything in your life, you don't need to do a hook-in check.
You're going to die anyway so you might as well fly. |

Of course, it is practical. I always hook the harness to the glider first, long before I ever get in it.
But do as you like. Far be if from me to tell you what to do. _________________ Davis Straub at the Oz Report |

I have been reading this thread and thinking about it for days now....
And i had an idea....
Why not require all tandom pilots to use a system that attaches the passenger harness to the pilots harness?
I am picturing a small carabiner near the main carabiner of each harness with a hangstrap type loop of material connecting the two together? Both harnesses would still hook in to the hangstrap like normal, but in a case of one hooking in and the other not, this could save both lives. You could even use locktight or something to make the connection permanent. Meaning a "dedicated" tandom harness. This along with both pilot and passenger hanging from SEPERATE hangstraps, would mean that 2 hangstraps would need to fail. (3 counting the backup) and BOTH pilot and passenger would need to be unhooked... and in that case the glider would fly away without them and at worse a tumble down launch. This could still be fatal but is less likely to be when compared with a "fall"
Also has anyone else noticed that the pilot didn't forget to hook in his own harness? I hate to even suggest this next part but, would it really be a surprise to find that a person was un-intentionally, subconsciously, more aware/concerned with his own safety, then the safety of another person.... i mean in a way that is human nature...
But this way it is an all-or-nothing situation... Since I am a noob, I am interested in hearing some experienced tandom pilots chime in.
---------------------------------------------------------------------------------------------------------
We already have such a system. However, no system will work without proper execution. IMO, this pilot failed to execute some or all of the safety procedures he had been taught and had been using until this one flight.
I feel for the girls family and friends. Sad indeed. _________________ MIke Jefferson

Do it "Triple" Hang Check then someone can double Check your Harness,
Chest Zip belt , both leg strap , steel carabiner Hook up to your Hang Glider
include two loops (main and back up) to your hang glider before "hook in" !
then next thing
Also ask someone can visual inspection your hang glider make sure everything
safety Include the steel carabiner has locked by thumb and double check on
their both leg strap , wrist belt inside harness, helmet chin sure locked position
help you hold the glider before launching then level wing and clear! to run off
Avoid Mistake to get happen !! |

Please support this cause, This is the young women that died hang gliding in a Hang Gliding Tandem accident in Canada, a trust fund has been set up in her name.........
(a number of Lenami’s close friends and family) have set up this memorial fund.
In her life, Lenami shared her desired to establish an award that would positively impact Mexican women. In honour of her passion and our love for her, we’ve worked to set up an award at Simon Fraser University.
Our goal is to raise over $20,000 initially. This will allow Lena’s memorial award to continue each year. We have so far raised 1/4 of the funds we need to create the endowment.Remembering Lena | Here to share and remember Lenami Godinez-Avila

Liftlover. What makes you think I have an illness? What is wrong about seeing the accident? We are not kids here and can be respectful about it,
and I can stay around here if I want dumbass. |

You didn't ask to see an accident- you asked to see the results. Adding to that, your question made clear you hadn't even bother reading the thread. You created the impression of only trolling for bloody entertainment -Liftlover's interpretation had cause. |
